Effective 5/1/18 all patients must communicate with the office through our patient portal and not through our website.
You may contact our office by registering on our patient portal and sending a secure message to our staff. We will contact you within 24 hours.
Our office cannot email records due to privacy concerns. Please contact our office and one of our staff will assist you in completing your request.
Please do not leave any urgent medical questions or requests for advice through the website effective 5/1/18.
If you are not contacted in 48 hours please call our office to speak to a receptionist.
